Drain Doctors Inc agrees to provide labor, materials, and equipment necessary to install (46) ZURN 5.0 inch cast iron no-hub roof drain with poly dome (Z-100-5NH) and (26) ZURN cast iron body, no-hub, with poly dome (Z-100-4NH). Project duration is 24 working days assuming an average of 3 drains can be installed per day. A 4 foot X 4 foot section of roof will be removed and reinstalled for each drain and will be performed by other contractors and is not included in Drain Doctors scope of work.

Drain Doctors Inc. will provide all material, labor and equipment to re-pipe sewers from buildings #8 and #11 to manholes. From where the sewers exit the buildings to the manholes there are major bellies or low spots in the lines which hold water and over a period of time and collects waste and other debris causing most of the stoppages occurring at these buildings.

Building #8: Utility locates must be called in to mark any conflicts we might encounter. Professional concrete cutters will saw cut and remove two sections of concrete walkway, curbing and asphalt as required. The debris will be hauled off the site. The sewers will be exposed by hand down to the existing 4 inch clean outs at the building and level the pipe out to a 1/16 of an inch fall per foot and install 65 feet of 6 inch SDR 35 sewer pipe. After inspection has been approved the trench will be compacted in 6 inch lifts to ensure that no future settling or bellies will occur. The 50 square feet of asphalt, 50 square feet of concrete and 6 linear feet of concrete curbing will be replaced.

Building #11: Utility locates must be called in to mark any conflicts we might encounter. Professional concrete cutters will saw cut and remove two sections of concrete walkway, curbing and asphalt as required. The debris will be hauled off the site. The sewers will be exposed by hand down to the existing 4 inch clean outs at the building and level the pipe out to a 1/16 of an inch fall per and install 105 feet of 6 inch SDR 35 sewer pipe. After inspection has been approved the trench will be compacted in 6 inch lifts to ensure that no future settling or bellies will occur. The 175 square feet of asphalt, 50 square feet of concrete and 6 linear feet of concrete curbing will be replaced. This proposal includes a full day of jetting and video inspection of the sewers under buildings #8 and #11 to determine if any further repairs are required.

Notes: *Barricades will be placed around both open trenches through duration of project and is expected to last one week. *This proposal does not include any grass or landscape restoration that will be required by digging. *This proposal includes permit fees and Notice of commencement which will need to be executed by owners

Drain Doctors Inc agrees to provide labor, materials, and equipment necessary to remove/reinstall fixtures for bathroom remodel. Store will be shut down for one week, with plumbing work occurring between the hours of 8am-5pm . Scope of work involves removing existing fixtures and cap lines while remodel is in progress. Other demo will be required and certain lines will need to be moved/properly abandoned if required. All fixtures will be removed to secure location (job box or POD provided by owner) and thoroughly cleaned. All fixtures will be reinstalled once remodeling is complete. Drain Doctors will only supply connection materials. All fixtures will be cleaned and reinstalled.

Drain Doctors Inc will provide all labor, equipment and material to install total of 5 clean outs around Building B. To install the two clean outs on the northern side of the property a 4 x 4 square will be cut in the asphalt and soil excavated down approximately 3.5 feet to the 4 inch terra cotta clay lateral. Once exposed a small section will be snap cut and removed. A 4 inch SCH 40 PVC two-way clean out with cast iron body will be installed and brought up to grade so future access to Building B's sewer main can be easily obtained. A total of 3 other clean outs are missing clean out covers and are not rated for a high traffic area. We propose to cut a 16 x 16 inch square around each of the three and expose the pipe enough to be able to secure a high traffic area clean out with cast iron body. The hole will be backfilled, compacted and high strength concrete laid. Cones will be set around all newly installed clean outs so concrete will have time to effectively cure. Once clean outs are installed a jetter will be ran through the vent stacks on opposite sides of the building toward the middle and then from the newly installed clean out on north side of the property, pulling back all debris from under the building. A video inspection will be performed though out this process to ensure jetting was effective.

Drain Doctors Inc will provide all material and labor to perform the following tasks. -Jett from second story roof between 8 to 10 vent stacks for the four units in the building. -Jett from cleanout on ground to remove debris from under building. -Record a DVD of the final video inspection and give to management company. -Provide detailed invoice of findings.

Notes: We will need to gain access to each unit to fill sinks with about 2" of water before jetting.

Drain Doctors Inc will provide all labor, equipment and material to install two 6" SCH 40 PVC two way cleanouts by north east corner of pool area and backfill to grade. The cleanouts will allow us to jet up stream and pull back all the debris and scale that is currently resting on the bottom of the pipe. During this process a camera will be used several times to ensure that we are thoroughly cleaning this line. Approximately 100' downstream of where the cleanouts are being installed there is a location where roots have penetrated into the pipe, probably where an old cleanout was located, we must excavate this area and install another 6" SCH 40 PVC two way cleanout. This additional cleanout will continue to allow us to jet upstream again pulling back all debris and scale from the 6" cast iron main line and letting it flow into the city.

After, all jetting is complete a video inspect will be performed again to verify a thorough job was performed and make any notations if any additional work is going to be required. Notes: During the initial video inspection performed on 4-14-08 we saw excessive amounts of build up and scale on the 6" cast iron main that runs along side of the south side of the property, however by installing cleanouts and jetting we can remove the debris and prolong the time in which this project must be performed. We recommend the pipe lining project stays in your future budget for this property, keep in mind that if the line does collapse it will not be able to be lined.
